Non-Medical Drivers of Health

PHIX’s regional focus and high participation coverage in the inpatient and outpatient healthcare ecosystem enables PHIX to provide a holistic view of patients across the community.

In addition to PHIX’s core services as a community health exchange, PHIX also collaborates with our partners and community leaders to improve community health outcomes by leveraging data and technology in other ways. PHIX’s Non-Medical Drivers of Health (NMDOH) Committee, focuses on how PHIX can strategically support integrating health and social determinant data together to support different initiatives.

PHIX Initiatives

Social Needs Screenings

Collect screening data to identify social needs

  • PHIX is working with our data sharing partners to ensure we are receiving social screening data to both support care coordination for individuals and analysis at the community level. More information on this initiative coming soon!

Holistic Care

Support sharing information on health care and social services within and across sectors at the individual level

  • PHIX is developing an electronic system and legal framework to support collecting and sharing data across sectors to support care coordination. More information on this initiative coming soon!

Coordination/ Referrals

Support coordination and referrals across sectors (e.g., health provider referrals to social services)

  • PHIX’s goal is to support secure, electronic process for sending referrals and receiving closed-loop communication when the patient is seen. This system was developed with generous funding from the Paso del Norte Health Foundation.

    The El Pasoans Fighting Hunger Food Bank has partnered with PHIX so providers can send closed-loop electronic referrals to their Food FARMacy, and PHIX is continuing to grow our closed-loop referral network.

High Utilizers

Identify individuals that leverage a higher number of services across sectors

  • Identify individuals that leverage a higher number of services across sectors.​ For example, PHIX is collaborating with the City of El Paso Fire Department’s Mobile Integrate Health (MIH) Team to support their work providing services for medical and social needs, including their initiative to support seniors at risk of falls. 

Community Analysis

Analyze community-level trends and needs to inform policy and program development

  • PHIX’s goal is to enhance clinical dashboards with non-medical drivers of health data to support understanding key health trends.
